| Required Information Regardless of the enrollment method used, the following information is required:

| Cancellations Enrollment requests are due no later than one week prior to the scheduled course start date. Scheduled courses with no enrollments by this time are subject to cancellation. Many courses fill early. Please notify Ed Kulak promptly of any enrollment cancellations. This will permit others to attend. |
